XT Tourniquet

The SAM XT Tourniquet’s effectiveness hinges on its integrated TRUEFLEX™ and TRUEFORCE technologies. TRUEFLEX™ is an engineering feature within the windlass system designed to distribute locking force evenly across its width. This targeted pressure application prevents the thin, high-pressure line that can cause nerve damage or fail on bulky extremities, creating a more reliable occlusion of blood flow. Crucially, this consistent force distribution enhances performance in challenging weather, as it is less susceptible to material stiffening in cold or slippage in wet conditions.

Complementing this, the TRUEFORCE mechanism utilizes an auto-lock technology. As the windlass rod is turned, the integrated locking prongs automatically engage, securely capturing each turn. This intelligent design actively eliminates all slack during tightening, a common point of failure in manual designs. The result is a rapid, single-handed application that achieves necessary occlusion pressure with minimal turns and maintains its locked position under stress, ensuring lifesaving reliability when seconds count.

The precision strap and hook & loop closure work in tandem to provide a secure, staged, and adaptable fit. The strap itself, with its pre-set interval holes, functions like a precision ratchet. It allows the user to thread the buckle through the most appropriate hole for the limb’s initial circumference, taking up the majority of slack before windlass twisting even begins. This provides immediate stability and prevents the tourniquet from shifting during critical final tightening.

Once the windlass is twisted and locked using the TRUEFORCE system, the long tail of the strap is then secured back over the mechanism via the hook & loop (Velcro®-type) closure. This secondary retention serves two vital purposes. First, it neatly seals the windlass rod in place, preventing it from accidentally catching on equipment or the environment and becoming dislodged. Second, it provides an additional layer of security, guarding against any potential loosening and ensuring the entire assembly remains intact during prolonged evacuation or transport.

The 1.5-inch (3.8 cm) width is the contemporary, evidence-based standard recommended by the Committee on Tactical Combat Casualty Care (CoTCCC) for all pre-hospital limb hemorrhage control, applicable to both adults and children. This width optimally balances effective occlusion of arterial blood flow with a reduced risk of local nerve damage compared to narrower designs. For pediatric use, it remains effective; proper placement high on the limb is paramount, and its adjustable design accommodates smaller extremities.

Regarding gear integration, the SAM XT’s streamlined profile is a key feature. The low-profile windlass and secure hook & loop closure are designed specifically to prevent snagging. Its distinct Orange/Black coloration allows for immediate visual identification in high-stress situations. The tourniquet can be quickly deployed from a pocket, medical kit, or dedicated outer-carrier pouch, and its standard width ensures compatibility with most individual first aid kits (IFAKs) and mounting systems, making it a versatile and ready component of any medical preparedness loadout.
